Golf Competition Requirements Your local coordinator should supply you with the following competition registration materials. A detailed Golf Competition Guide is available.   Prior to a competition, the following forms must be submitted: Delegation Information Non-Athlete (Coach/Chaperone) Listing Athlete Sport Entry Form Med Cards Golf Score Cards if no established handicap Partner and Unified Team Forms Expect to complete the following on-site forms for competition registration: Housing Forms Scratch List Registration Summary Form
